The Dismal Success of Anti-Cancer Treatments


A placebo is a term that describes the administration of a sugar pill or dummy procedure in order to test whether a drug or procedure is more effective than the power of belief. In an article in the Guardian (Thursday, June 20, 2002), Jerome Burne reported that "new research suggests that placebos work surprisingly well, ­ in fact, rather better than some conventional drugs."
A placebo (which literally means, "I shall please") is included as an indispensable element of every scientific study conducted today. The placebo effect is purely based on the subjective feelings of a person. Each person who is tested for the efficacy of a medical drug believes in the drug in a unique and unpredictable way. A certain number of people may have a hopeful, trusting disposition and, therefore, a stronger placebo response than others. Others may be suffering from depression, which is known to affect a person's ability to respond positively to any kind of treatment. As a result, one study may "prove" a particular drug to be effective for, let us say, a certain kind of cancer. However, if a repeat experiment is conducted with different subjects, this drug may turn out to be ineffective when compared to the placebo response.
For this reason, pharmaceutical companies instruct their paid researchers to publish only the most favorable findings from these various experiments. Those parts of the study where the drug has had no or only an insignificant advantage over the placebo effect are simply omitted from the study's final report.
The drug companies reporting their findings to the Food and Drug Administration (FDA) only need to prove that the tested drug has shown some benefit in some people. If the researchers manage to recruit enough candidates with a positive disposition that are likely to produce a good placebo response to the drug treatment, they may hit the jackpot and produce a "convincing" study, and a marketable drug.
This is a no-brainer for drug makers since FDA approval is granted to anti-cancer drugs based on response rates that are at best in the 10-20 percent. In addition, the "success" of most clinical cancer studies is measured by tumor shrinkage instead of mortality rate. In other words, even if most of the subjects died but had their tumors shrunk through aggressive treatments, the study would be hailed as a great success and a medical breakthrough.
Any such attempt to treat the human body as if it were a machine that just responds to mechanical or chemical manipulation is bound to have serious setbacks. Such an approach is not only unscientific, but also unethical and potentially harmful. For many cancer patients whose immune systems are already compromised, just one dose of chemotherapy or radiation can turn out to be fatal.
Senior cancer physician, Dr. Charles Moertel of the famous Mayo Clinic in Rochester, Minnesota, once aptly summarized the modern cancer treatment dilemma in the following words: "Our most effective regimens are fraught with risks and side effects and practical problems, and after this price is paid by all the patients we have treated, only a small fraction are rewarded with a transient period of usually incomplete tumor regression."
The success record of modern cancer therapy is dismal, significantly less than even the weakest placebo response. On the average, remission occurs in only about 7 percent of cancer patients. Moreover, there is no evidence that this discouragingly low 7 percent "success rate" results from the treatments offered; it could just as well be in spite of the treatments. This is more likely, since not treating cancer at all has a much higher success rate than treating it. A drug treatment that promises temporary tumor shrinkage in 10 percent of patients is not a promising therapy; rather, it is a dangerous gamble with their life.

Future Treatments for Breast Cancer


Standard treatment for breast cancer has greatly improved survival for women, but the fact remains that 40% of women still ultimately die from the disease. This fact highlights theneed for new and better therapies. Traditional anticancer therapies mostly consist of drugs and ionizing radiation that damage DNA and DNA machinery. Such treatments preferentially kill certain kinds of cancer cells because these cells have a diminished ability to survive the damage. However, normal cells, can suffer DNA damage, but have the ability to repair their DNA. Unfortunately, the same genetic defects may render some cancer cells resistant to treatment, since they may also be less adept at activating cell death in the face of DNA damage. The challenge of cancer treatment now is to take advantage of properties of cancer cells that distinguish them from normal cells and to subsequently and specifically target cancer cells. A growing understanding of cancer cell biology is leading to better methods for treating the disease by directly exterminating only cancer cells. Listed are some of the latest new and adventurous strategies that are being developed for future breast cancer diagnosis and treatment.
Six new breast cancer treatment strategies
1. Oncolytic viral therapy - takes advantage of molecules that tumor cells lack. Tumor cells usually lack the protein p53 which controls the cell cycle and cell replication. Viruses can attach to a host cell and replicate continuously inside the cell. They then burst out when their numbers are sufficient, killing the cell and infecting its neighbors. An adenovirus has now been constructed that lacks the gene for the p53 protein; this virus can therefore only replicate in cells in which p53 is already inactivated-including many types of cancer cells. If this modified adenovirus is injected into a tumor, it will replicate in and kill only the cancer cells that lack p53, leaving normal cells unharmed. This strategy is presently undergoing clinical trials.
2. Interventional Radiology - Scientists are investigating a technique in which magnets are used to pull chemotherapy drugs into tumors. Microscopic magnetic particles are attached to the cancer drugs and infused into the blood vessel that feeds the tumor. A rare earth magnet is positioned directly above the tumor site. The magnet pulls the drug-carrying particles out of the blood vessel directly into the tumor. This technique is still experimental; however, early research is promising. Scientists hope that it will bolster the effects of chemotherapy and avoid drug side effects.
3. Gene Therapy - involves the alteration of the patient's genetic material to fight or prevent cancer. Although the science of gene therapy is still in the experimental stages, researchers hope that in the future the therapy can be used as follows:
  • Alter the cells of a patient's natural immune system with cancer-fighting genes that allow cells to more forcefully attack the cancer;
  • Genetically alter cancer cells so that the patient's own immune system will defend against them and allow the altered cancer cells to act as a cancer vaccine;
  • Replace genes responsible for cancer growth with "good" genes; and inject a tumor with genes that will make it more susceptible to cancer-fighting agents.
  • Make normal tissue resistant to chemotherapy, so that drugs will destroy tumors without damaging healthy tissue.
4. Breast Cancer vaccines - Vaccination strategies are being developed that engage the patient's own immune system to eradicate cancer cells. The advantage of this is low toxicity, and high specificity as well as a sustained antibody effect due to cell memory.
5. Use of breast cancer resistance genes - Researchers at the Dana Faber Cancer Institute have found a gene that can predict a high risk of cancer recurrence in some breast cancers that have been treated with chemotherapy drugs. This finding could guide the physician in the choice of anti-tumor drugs to use and eliminate wrong ineffective choices.
6. More precise breast cancer diagnosis - Breast cancer can now be characterized at a molecular level by obtaining expression profiles for thousands of genes within the cancer. These profiles can be grouped into subclasses of breast cancers that share common features such as prognosis and probable response to certain treatments. More precise treatments can then be made for each subclass of cancer

Screening refers to the various tests and examinations that are carried out on the breasts of a person to detect the disease at the earliest, even before the actual symptoms of the disease starts to show. Usually breast cancers that can be felt and detected are cancers that have grown larger in size and that which in usual cases has spread to regions outside the breast. The treatment in such cases is very complex. Breast cancers can be treated successfully if it is detected very early, and before the actual cancer symptoms starts to show.
Breast cancer treatment
Breast cancer is treated either through local treatment or systemic treatment.
Local treatment of breast cancer involves removal, destruction, or controlling the growth of cancerous cells in the breast. Local treatment by surgery is also known by the names mastectomy, and it is also referred to as lumpectomy. This is a breast preserving therapy. The other local treatment is the use of radiation therapy. Radiation therapy involves use of a high energy radiation beam to destroy the cancer cells. Advanced equipment like LINAC helps in focusing the radiation beam on to the cancer cells, and minimizes, or avoids causing damage to the surrounding healthy cells.
Systemic treatment becomes necessary if the breast cancer has spread to other parts of the body. This treatment is therefore used to control and destroy cancer cells across the body. The various therapies that are used in this treatment include chemotherapy, hormone therapy, and biological therapy.
Chemotherapy involves the use of drugs to kill or control cancer cells. Hormone therapy involves use of certain drugs that inhibit production of certain hormones in the body, like estrogen, which can aid in the revival of cancerous breast cells that manage to remain in the body post the surgery of the breast. Biological therapy involves use of the body's immune system in fighting and destroying cancerous cells.

The symptom that is commonly associated with lung cancer is shortness of breath and presence of wheezing. Imaging techniques like X-Rays of chest, bronchoscopy, MRI, CT, and PET scans, are also used to diagnose lung cancer. Further, the patient is also subject to a physical examination by the physician, and an analysis of the patient's sputum is also made. All these examinations and procedures will reveal the presence of lung cancer, and also the location of the tumor, and also information about the presence or absence of the disease anywhere else in the body.
Lung cancer treatment
There are many factors which influence lung cancer treatment, and the factors on which lung cancer treatment is dependent on includes, the type of the cancer, the stage or how far the cancer has spread, the age of the patient, his or her health status, and many other personal characteristics of the patient. We are not looking at a potential cure here but merely at an extension of life. The ability of cancer cells to become resistant & to return with complete immunity to this drug means it does nothing more than 'bide us more time'.

  • PARP Inhibitors
Early results in the use of PARP inhibitors for treating breast cancer are said to be promising. PARP stands for poly(adenosine-disposphate-ribose) polymerase, an enzyme which repairs damage done to our DNA.
These drugs are said to work by "obstructing the ability of cells damaged by chemotherapy or through genetic mutations to repair themselves, causing tumour cells to die as a result" (WSJ).
PARP is found in all cells, including cancer cells & helps damaged cells to repair themselves. According to some doctors, because cancer cells have BRCA gene faults, if they can stop the PARP-1 working then these cells will be unable to repair themselves & will die. As healthy cells do not contain BRCA gene faults they should still be able to repair themselves despite the fact that PARP-1 is blocked in all cells of the body, not just cancer cells.
This treatment is still in the early trial stages, but could be a successful treatment for cancers with an abnormal BRCA1 or BRCA2 gene. This drug, when used in conjunction with chemotherapy may reduce the resistance of cancer cells to the chemotherapy. This treatment is only currently available to those with an abnormal BRCA1 or BRCA2 gene being treated for advanced-stage breast, ovarian, or prostate cancer through the participation in clinical trials.
Known Side Effects
As this is such a new treatment little is yet known about side effects, however there are a few that doctors do already know about; Drop in Blood Cell Count - Causing tiredness, shortness of breath, an increased risk of infection or bleeding problems.
Conclusion
Although it is stated that inhibiting PARP-1 should not affect the functioning of healthy cells this quite simply cannot be the case, as is shown in the fact that this can cause a drop in blood cell count. Surely this is evidence of damage to blood cells, which are unable to repair themselves? I find it very hard to believe that PARP would be found in all healthy cells if it was not in fact needed by them as part of their healthy functioning. We should be increasing the strength of our healthy cells & immune system, not obstructing their ability to repair themselves, common sense tells us we are just weakening our entire body in the hope of killing the cancer cells before the damage to our healthy cells kills us first. Cells are the making of our entire form, our bones, our blood, our skin as well as our internal organs, by denying them of something which they to maintain themselves we just leave ourselves wide open to infection & further illness.
  • Herceptin (trastuzumab)
Another drug used against breast cancer & also stomach cancer; apparently herceptin can stop the growth of breast cancer & in some cases can even reduce the size of the tumour. Herceptin is only used either in conjunction with chemotherapy or by those who have already completed two courses of chemotherapy.

Fatigue - this can last anything between 6 months - 1 year after treatment, although gradually decreases over time.
Flu-like Symptoms - Nausea, sickness, headache, fever & chills may be experienced & are said to subside after a few hours following treatment. These side effects are usually counteracted with further medication.
Allergic Reaction - Affects around half of patients, usually reduces across further treatments. Symptoms include fever, chills, skin rash, hives, itching, wheezing, headache, sickness, flushes, faintness, breathlessness & difficulty breathing.

Considering the claims that this drug causes 'little harm to healthy cells,' when you look at the above side effects, this is clearly put into perspective. Anything which produces side effects like this & more commonly according to some reports than widely published, is harmful & damaging to our cells & our long term health.
Another strong-point; this drug is only used with or after chemotherapy. Chemotherapy causes significant damage to our body & kills more people than cancer. Surely to use this in the first place is beyond all reason, let alone in conjunction with another drug with such awful side-effects like almost any one of the above! Everyone knows the poor quality of life which results from chemotherapy, especially from repeated treatment, I can only dread to think about the suffering people may endure through a combination of chemotherapy & herceptin.
A fair few of the side effects mentioned are treated with 'further medication.' Further discomfort & damage to our body can be the only real result of this, even if initial relief is felt.
Medical Industry figures indicate a 50% increase in the survival rate of those using herceptin against breast cancer; unfortunately just more misleading statistics. The true figure is just 12%.

One example of this is the 40-50% survival rate of chemotherapy patients, what is not always made clear is that this is only over a five year period. If you were to take into account how many people live the rest of their lives completely cancer free & never die of cancer, the treatment, or through any indirect cause from the treatment, this figure is just 3%! This is due to the increased susceptibility to cancer that chemotherapy causes, far more often than not it does return in the future & this time the chances of survival are far less.
In this day & age & with the advanced levels of science, is this the best they can come up with?
